Animal Health Certificate Request Form
Please find below animal health certificate questionnaire that we kindly ask you to fill out and submit to us prior to your appointment. We also ask that you please scan and email copies of your rabies vaccination and send them to the practice at least 48 hours before your scheduled appointment so that they can be attached to the certificate ahead of time. Please bring the original rabies certificates or pet passport with rabies vaccination proof with you at the time of your appointment in case we need to make additional copies. This ensures that the process goes smoothly. Please note Animal Health Certificate appointments need to be within 10 days of travel.

Tapeworm treatment is required when travelling with dogs (not cats or ferrets) from Great Britain to EU listed tapeworm free countries including Northern Ireland, Malta, Ireland, and Finland. These dogs must be treated for tapeworm by a veterinary surgeon not more than 120 hours (5 days) and not less than 24 hours before the time of entry into one of these countries and the treatment certified in the relevant section of the AHC.
